I played through knee pain for seven years. Ten physios, injections, a surgery that cost me a season. Nobody once explained the thing that actually mattered.
If you're reading this, you already know the pain I'm talking about. Not the whole knee. One spot, just below the kneecap. It shows up the moment you jump, land, or plant to change direction — and it goes quiet the moment you stop.
You've probably been told to rest it. Maybe to stop entirely. And every time you came back, it was waiting in exactly the same place.
I want to explain why that happens, because it took me the better part of a decade to find out, and it turned out to be simpler than anyone made it sound.
I've worked with numerous PTs, tried PRP injections, NSAIDs. MRI and ortho tests all came back showing no afflictions — which sounds like good news right up until you're the one whose knee hits a 10 out of 10 almost daily and gets told there's nothing wrong.
That's the part nobody prepares you for. The scans are clean. The tests are normal. And you still can't get through a session without it flaring.
If you've been made to feel like you're exaggerating — you're not. A tendon can be doing exactly what it shouldn't and still look unremarkable on a scan. Clean imaging doesn't mean the pain isn't happening.
Here's what I eventually understood about that loop, and it reframed everything:
Rest was never going to fix it. It was answering the wrong question.
Rest pauses the load. It doesn't change anything about where the load goes when you start moving again. So the tendon gets a break, feels better, and then the very first time you jump, the strain lands right back on the same square centimetre it always did.
It's not that you didn't rest long enough. It's that resting longer was never the variable that mattered.

Every one of those made sense when I tried it. Not one of them changed anything, because not one of them addressed where the force lands.
The person who finally explained it wasn't a specialist. He was a trainer who'd worked with a pro team, and he asked me one question nobody else had.
Not how bad does it hurt. When does it hurt.
Every time I jumped. Every time I landed. Every time I planted to cut. Same spot, every time. I said yes, obviously — and he said that was the answer.
Here's how he put it. Every time you jump or land, the load travels down and funnels into the same small point of the tendon, at the same angle, every single time. It's the repetition of angle, not just the amount of activity, that keeps the same spot irritated.
Think of a rope running over an edge. It frays at one point — the point where it bends. You can stop pulling for a week, and the fray doesn't move. The second you pull again, the tension lands on the same worn spot. Resting the rope was never the fix. Changing where it bends is.
If the problem is where the load lands, then the fix is anything that changes the angle at which the tendon gets loaded.
That's what a strap worn just below the kneecap does. It sits on the tendon and shifts the angle it's loaded at — so the load stops concentrating on that one sore spot while you move.
And here's where I have to be straight with you, because by then I'd been sold enough "this'll fix it" to fill a drawer:
It does not heal the tendon. Nothing you strap on heals a tendon. It changes where the load goes while you move — so you can play with less pain now, instead of sitting out waiting for a "someday" that rest kept not delivering.
That distinction is the only reason I believed it. Everything else I'd been offered promised to fix me. This one just claimed to move the load somewhere less angry, during the exact window I actually needed help — while playing.
Fair question. I'd stopped trusting explanations by that point too.
The biomechanics have been measured. Research on infrapatellar straps found the strap decreases localized strain at the tendon by increasing the patella–patellar-tendon angle — the technical way of saying it changes the loading angle. Notably, the same research is clear that it does not correct any anatomical defect. It changes load geometry. That's all it claims to do, and that's what it does.
That's not "cured." It's not "pain-free forever." It's less pain while you play, measured while actually playing. Which, if you've been stuck in the rest-and-repeat loop, is the thing you've been asking for the whole time.
I did, actually. Twice. Here's what happened both times.
The cheap ones are fabric. Fabric stretches. Within ten minutes of real movement it had slid down my leg off the spot it was supposed to be on, and at that point it's just a band around your knee doing nothing in particular. The pressure on the tendon isn't strong enough, and it does not stay in place.

The strap I ended up with is molded silicone rather than fabric. It grips the leg instead of relying on a tight squeeze, which is why it holds its place through cutting and jumping rather than rolling down mid-game.
